Is a BBL worth it? Results, how long they last, and regret

Only you can decide whether it's worth it. What we can do is lay out what the evidence says about results, what changes them, and what people say they wish they'd known.

At a glance

Part of the fat moved in a BBL doesn't survive. Small studies of the above-muscle technique report around 80% of the injected volume remaining at one year, but results vary widely between patients and studies. The fat that survives behaves like the rest of your body fat, so large weight gain or loss changes the shape. There's no reliable large study of BBL regret. Decide with realistic photos, a clear budget and a plan for recovery.

How much fat survives

Transferred fat has to grow a new blood supply in its new place. Some of it doesn't make it, and the body absorbs it over the first months.

  • A small study presented at the American Society of Plastic Surgeons' 2018 meeting measured the buttock fat layer by ultrasound and estimated that about 82% of the injected volume remained at 12 months with above-muscle injection.
  • Other studies of fat grafting in different parts of the body report lower and very variable figures.

Do the results last?

The fat that survives the first months generally stays. It is still fat, though:

  • Weight gain can enlarge the buttocks along with the rest of your body. Fat may also return to areas that had liposuction, often in different places.
  • Weight loss can shrink the result.
  • Pregnancy and aging change skin and shape over time.

That's why surgeons prefer patients at a stable weight they can keep. Am I a good candidate?

Regret and satisfaction

There is no reliable large study of BBL regret, and we won't guess at a number. In Miami patient groups, the regrets people describe most are about the process, not only the result:

  • choosing on price or a promo, then finding the quote didn't cover what they needed. What surgery really costs
  • not verifying who would actually operate. The 15-minute check
  • underestimating recovery: weeks without sitting, help needed, time off work. Time off work
  • judging the result in the first weeks while swollen, or booking a revision too soon.
If you're struggling after surgery

Deciding

  1. Ask to see results on patients with your build, six months or more after surgery.
  2. Understand the specific risk of a BBL before you book. Is a BBL safe?
  3. Plan the whole recovery, not only surgery day.
  4. Give yourself time. A deposit deadline is not a medical reason to decide quickly.

Usually several months after surgery, once swelling settles. The buttocks often look bigger in the first weeks than they will at the end, which is expected.
